You are only eligible for a passport renewal if ALL of the following are true:
Your passport is currently valid, or has expired within the last five years and
Your passport is currently in your possession and
Your passport is not damaged in any way and
Your passport was issued when you were age 16 or older.
You may request a passport renewal for any one of the following reasons:
Your passport is expired or will expire soon.
Your passport is low on blank visa pages.
You have legally changed your name and your passport was issued more than one year ago.
What documents do you need to prepare?
One completed DS-82 application (hand-written applications are not accepted)
Two identical passport photos taken within the last 6 months (do not wear glasses or hats/face coverings).
Your expired or expiring passport (must be in good condition) and an original or certified copy of a name change document (marriage, divorce, adoption, etc.) if your name has changed since your expiring passport was last issued.
A check made out to "US Department of State" for $209.53 Your name and address must be in the top left-hand corner and your full name and date of birth must be written in the memo section.
A travel itinerary (airline tickets, cruise or hotel reservations, etc.). A business letter on official company letterhead can be used in lieu of a travel itinerary for business purposes.
Two (2) copies of the Letter of Authorization per applicant. For example, if Tommy and Jimmy both need passports, include four (4) total letters of authorization. Please check the first box.

You are eligible for a new passport if ONE of the following is true:
You are applying for your first U.S. passport.
Your previous U.S. passport was issued when you before your 16th birthday.
Your last-issued U.S. passport was issued more than 15 years ago.

A completed DS-11 application (hand-written applications are not accepted). Please print a total of two copies. DO NOT SIGN UNTIL YOU ARE IN FRONT OF THE ACCEPTANCE AGENT!!! IF A BARCODE IS NOT IN THE TOP LEFT-HAND CORNER, YOUR APPLICATION WILL BE DENIED!
Two identical passport photos taken within the last 6 months (do not wear glasses or hats/face coverings).
An ORIGINAL or CERTIFIED copy of your birth certificate OR an ORIGINAL naturalization certificate OR an expired passport AND a marriage certificate or divorce certificate is required if the applicant's last name has changed.
A valid copy of an ID that has been issued for AT LEAST 6 months. A valid ID can be a driver's license, state ID, military ID, or any Gov ID. Learner's permits and temporary IDs are not valid.

You are eligible for passport name change service if:
Your current passport is valid, in your possession, and undamaged or
You have legally changed your name since your passport was issued and
You are able to submit an original proof of name change (marriage certificate, divorce certificate, or court order).
What documents do you need to prepare?
A completed DS-5504 if your passport is LESS THAN 1 year old or a DS-82 if it was issued MORE THAN 1 year ago. (hand-written applications are not accepted).

Your damaged or mutilated passport (must be included within the sealed envelope that you will receive at the acceptance facility. This passport can not be used for citizenship verification and will NOT be returned to you).

If you are traveling between politically sensitive countries (ex. Israel and Saudi Arabia)
If you are a very frequent traveler and need to obtain a visa stamp in your currently issued passport passport while you are traveling abroad.
