NEW PASSPORT CHECKLIST:

*This service requires that you first visit a Passport Acceptance Facility*

You are eligible for a new passport if ONE of the following is true:

You are applying for your first U.S. passport.

Your previous U.S. passport was issued when you before your 16th birthday.

Your last-issued U.S. passport was issued more than 15 years ago.

 

PASSPORT RENEWAL CHECKLIST

You are only eligible for a passport renewal if ALL of the following are true:

Your passport is currently valid, or has expired within the last five years and

Your passport is currently in your possession and

Your passport is not damaged in any way and

Your passport was issued when you were age 16 or older.

You may request a passport renewal for any one of the following reasons:

Your passport is expired or will expire soon.

Your passport is low on blank visa pages.

You have legally changed your name and your passport was issued more than one year ago.

 

CHILD PASSPORT CHECKLIST:

Please use this service if the child is under 16 years of age, even if the child has previously been issued a passport.

 

STOLEN PASSPORT CHECKLIST:

*This service requires that you first visit a Passport Acceptance Facility*

Use this service if you most recently issued passport was lost or stolen (even if it is or was expired)

 

LOST PASSPORT CHECKLIST:

*This service requires that you first visit a Passport Acceptance Facility*

Please use this service if your most recent passport was lost or stolen, even if your passport was expired.

 

NAME CHANGE CHECKLIST:

You are eligible for passport name change service if:

Your current passport is valid, in your possession, and undamaged or

You have legally changed your name since your passport was issued and

You are able to submit an original proof of name change (marriage certificate, divorce certificate, or court order).

 

DAMAGED PASSPORT CHECKLIST:

Please use this service if your most recent passport was severely damaged.

 

SECOND PASSPORT CHECKLIST:

Please use this service only if you meet one of the following criteria:

If you are traveling between politically sensitive countries (ex. Israel and Saudi Arabia)

If you are a very frequent traveler and need to obtain a visa stamp in your currently issued passport passport while you are traveling abroad.
